5. Latest Financial Audit Annual Statement
6. Form 83 (for branch office).
7. Latest E-Filing + receipt (for companies operating more than 1 year).
8. Latest Company Bank Statement
9. Support Letter from Regulatory Agency (for restricted industry only).
10. Latest Project Documents (LOA / PO) (common for engineering industry).
11. Organizational Chart (with the applicant's position).
12. Company Logo (.png .jpg or .jpeg).
13. Company Representative IC (management position or higher. eg. HR executive, manager, director).
